• Visit www.google.com/maps. A new window will open.
• Make sure you're signed in to your Google account (if you have Gmail, this is easy!).
• Search for a Provider or their practice.
• Click Write a Review.
• In the window that appears, click the stars to provide a score. You can also write a review.

WebMD

• Select the image to the right or www.webmd.com. A new window will open.
• Click on Find a Doctor.
• Enter the provider's name or practice and location.
• Once results appear, click on the provider's name
• Click on Rate this Doctor.
• In the window that appears, click the stars to provide a score. You can also write a review

Vitals 

• Select the image to the right or www.vitals.com. A new window will open.
• Search for a Provider or their practice.
• Click Write a Review.
• In the window that appears, click the stars to provide a score. You can also write a review.

Healthgrades 

• Visit www.healthgrades.com/doctor-reviews. A new window will open.
• Type in the name of your Provider.
• If they are in the Healthgrades system, a Patient Satisfaction Survey will appear.

RateMDs   

• Visit www.ratemds.com. A new window will open.
• Type in the name and location of your Provider.
• Results should appear. Scroll down to where you can complete a review.
